If you change one field on your profile this month, make it this one. The primary category is the strongest relevance signal you control, and it acts as a gate: it determines which searches you are even eligible to appear in.
Why the primary category dominates
Google treats the primary category as your fundamental answer to what kind of business you are. Secondary categories broaden you, but the primary carries disproportionate weight in deciding whether you surface for a given query.
The practical consequence is that a practice with the wrong primary category can do everything else perfectly and still be invisible for its most important searches. It is not competing badly, it is not entered.
The realistic options
Google maintains a fixed category list and you cannot invent entries. There is no category for several treatments you may provide, which surprises most owners. The credible primary choices are typically:
- Psychiatrist. The default for most practices led by a psychiatrist. Broad, well understood, competitive.
- Mental health clinic. Suits a multi-clinician practice presenting as a facility rather than an individual.
- Mental health service. Broader and vaguer. Usually a weaker choice than the two above.
- Medical clinic. Too general for most, but occasionally right for a practice where psychiatry sits inside a wider medical offering.
- Psychologist or counselor. Only where that reflects the actual licensure of the practice.
Note what is missing: there is generally no primary category naming a specific modality. That work is done by secondary categories and the services list, which is the next lesson.
How to choose
Three questions, in order:
- What would a patient call you? The category should match the plain-language description a patient would use, because that is what they type.
- What are your competitors using? Look at the practices ranking for your target searches and note their primary category. If every one of them is Psychiatrist and you are Mental health service, that gap is informative.
- What does your licensure support? Never choose a category implying credentials the practice does not hold.
For most interventional psychiatry practices, Psychiatrist is right for a physician-led clinic and Mental health clinic is right for a larger multi-provider facility.
Changing it later
You can change the primary category, but treat it as a considered change rather than an experiment:
- Edits to a healthcare profile can trigger review, and the profile may be limited while that happens.
- Standing for existing searches can shift, sometimes downward at first.
- Repeated changes look like manipulation.
Record the date of any change and give it several weeks before judging the result. Do not change the primary category and the practice name in the same week, or you will not know which caused what.
A mistake worth naming
Do not pick a niche category to reduce competition. A practice choosing an obscure category because it seems less crowded ends up eligible for searches nobody performs. Being tenth for a term with real volume is worth far more than being first for a term with none.
